And I printed this sheet out and I took it with me every time that I observed. Um, and I had written on it, so, it kind of looks something like this, um, I had the where I observed, the date, the time that I started my observation and the time my observation ended, I had the name of the therapist and the name of the facility, and then I also had the signature of the therapist, so I had them sign at the end of every single time that I observed.
